Appointments for Clients under 18

If you are under the age of 18, you must bring the following with you to your appointment:

• Your parent or legal guardian (they must bring their ID to the appointment)
• Your ID
• Your birth certificate. The names on your and your guardian's IDs must match the names on your birth certificate

Appointments always available, walk-ins always welcome.
